    Help with Estrace cream dosage....

    I'm menopausal and have been suffering with vag pain and frequency for years. I've been dxed with IC & OAB but none of the meds helped in the slightest. I learned about the importance of hormones to the urinary tract here and when I brought up the possibility, my gyno agreed. He put me on Estrace cream and told me to apply a small amount to the vulva/urethra every 3 days. I used it yesterday for the first time and experienced 4 blissful hours of relief...then the irritation, discomfort & pain returned.

    I would love to use it more often but don't know what the parameters are for safe dosage. My gyno is very stingy with info & doesn't really explain very much (and, to put it mildly, I am enraged that he never once brought up the possibility of hormone treatment for the vaginal & urinary problems I've been complaining about for years)...I will certainly make a change but until I can find a new gyno, I hope I can get some more detailed help here.

    Is the goal to restore vag estrogen and then to reduce usage? It seems to me that a small amount every 3 days is inadequate but perhaps the gyno's idea is to rebuild the estrogen levels slowly. Does anyone know?

    Dear Systitis -

    Normally, you use the Estrace Cream by putting in 1 mg/gm in the vagina every day for two weeks then 2-3 times per week after that.

    I am on estrace cream once a day & vagifem twice a week. It's made a big difference for me, taking away the 24/7 burning. I use a fingertip amount of the estrace. In fact, I was wrongly diagnosed with VV & it was all atrophic vaginitis. It sounds like it is time to find a new gyn.

    When first prescribed, I had to use 2 grams every night for 2 weeks, then back off to 2 grams every-other-night for 2 weeks, then 2 grams 3 nights per week. I am currently using 1 gram 3 nights a week.

    Estrace

    I am currently using Estrace cream 0.1 gram either three times a week or 0.2 grams twice a week internally, and smeared around urethra and vulva.
    I also take estrogen replacement with pellets inserted under the skin, used to take bio-identicals 2 mg daily.
    Boy, if I dont have my estrogen--I know it.

    Have you had or do you have any bleeding? I tried the Estrace and had a pinkish bleeding one time so I stopped using it...I'm cautious but want relief...

    Bleeding

    Your best bet is to get a pelvic exam from your Gyno. The bleeding may or may not be related to your insertion of the hormone cream. If you were very atrophic anything would cause a slight discharge as you would be very inflammed, it could also cause temporary burning as the nerves regain their function and moisture from the estrogen. But--only a physical exam could tell you whats going on up there. Do you have a history of spotting on inter cyclic bleeding? If it were me, and I had not history of spotting I would go in for a pelvic exam.

    Hi,

    I was wondering how long it can take to notice the effects from Estrace cream? I have been on .5g once daily for 2 weeks (then .5g 2x week thereafter). It is now 2 1/2 weeks and haven't noticed a difference really in my vaginal pain. My problem is believed to be nerve-related as everything else has been ruled out. So just curious if it can take longer than the 2 week period to notice changes?

    That is an extremely low dose, Micky. I started out using 2 grams every night for 2 weeks, then 2 grams every-other-night for 2 weeks. I am now down to a maintenance dose of 1 gram three nights per week.

    My Gyn prescribed it for vaginal atrophy which was causing me to have UTI's every two weeks. It's been over a year now since I initially began using Estrace Cream, and I haven't had any UTI's at all.

    It still hurts to have a vaginal exam, so everything is obviously not 100%, but at least the UTI's have stopped.

    vaginal atrophy



    I was diagnosed with vaginal atrophy 3 years ago. This was before my symptoms started. I was put on 1g. of Premarin once a week. Two years later my symptoms of urinary frequency and urgency and throbbing pain in the clitoral area began. Two urogynecologists told me that I was not taking enough of the Premarin. The one I went to this week-a top specialist at NYU, put me on Premarin 1g. every night for a month, then every other night for a month. Then i have to see him. So I am anxious to see if this helps.

    I had blood issues, too, as a post menopausal woman with very very dry genital tissue which had expanded up into the lower bladder.

    I would, sometimes, have some blood on the TP when I wiped and a few times in the bowl when I urinated.

    1 gm of Estrace 3 x per week has solved all my problems.

    Please be careful with Estrace; it gave me an incredibly painful and inflammed vulva. Vagifem works much better for me.
