Does anyone have problem arousal during sleep that begins as if an orgasm is going to follow, but stops at the engorgement stage and leaves the person in pain and irritation to the whole vulvar and urethral area as well as exacerbating I.C. pain in the bladder. I have read that certain antihistamines such as Trazadone may cause this state to occur. Also seems possible that pudendal nerve irritation might cause it. Thank you. Lyse Magness

Briza, thank you so much for giving me the site to follow others' posts about hyperarousal and clitoral irritation. I was away from using the ic network for quite a while and missed those when they came out, which seemed to be mainly in 2007. It seems a number of people got some help from amitriptyline, which is encouraging to me because it is helpful in helping me sleep, and I had read where certain antidepressants, namely Trazodone, were believed to cause the hyperarousal or female priapism, which made me afraid to use amitriptyline is a tri-cyclic type antidepressant and may be different in some important way from Trazadone. I explained to my doctor last week again about how the clitoral problem starts up while I am asleep beginnning as a pleasurable sensation like I am about to come to orgrasm, but gets hung up at the engorgement stage and no orgasm or relief of any kind comes--instead severe pain and irritaton that goes on for one or two days or more. He said to try Valium inserted in the vagina. He apparently feels it is happening because of pelvic floor dysfunction. I tried one Valium last night, but instead of any relief it seemed to irritate my urethrat and the whole vaginal area and cause burning and urinary retention at first followed by the urge to urinate every 10 minutes or so. I tried putting the tablet as far into the vagina as I could, but apparently some of it leaked out and got to my urethral opening. I had had a similar thing happen when I tried to use an anti yeast capsule inserted in the vagina. Almost any chemical seems to set off pain for me if even a small amount gets to the urethra. I am going to try getting the empty capsules and crushing the Valium maybe cutting it into 5 mg size, but I still don't see how I can keep some of it from getting to the urethra. I did not have the engorgement pain but this other pain and irritation was almost as aggravating. I wonder if anyone currently has another suggestion about how to prevent this from happening or how to treat it once it has started. thank you. Lyse

Hi Lyse,
I have this sometimes, I did have it often the clitoral pain was the worse. I am now on herbs to downregulate the nervous system which has decreased the level to which it happens, but it still happens.
I tried Elavail, didn't work for me.

I also have problems with things getting near the ureathra - mainly soap but I can't use anti yeast creams at all as it makes my vv worse. Currently not wearing any underwear for a few days and only washing with water seems to help. A suggestion would be to get the diazapam compounded in a lipoderm - I think thats what its called (non irritating) base. Pure dizapam has no color in it so won't irritate.

Thank you, Claire A., I am going to try the compounded form of the Valium.
