Just some quick questions for all of ya--during the last hydros I have had no concurrent instillations. Of course self cathing with marcaine and heparin seems awesome. Rhonda, I know you do it, does it really hurt. Would love to chat about it in depth. Thanks for listening and understanding. Wed is arriving soon

After I have a hydro, I spend overnight in the hospital for a continuous irrigation containing solu cortef, then go back to DMSO two weeks later.

Hi Barb,
After he did my hydro, my uro instilled DMSO into my bladder while I was still under. That counted as my first treatment -- and it was the only treatment that I actually didn't really feel any pain for. I had gotten a lot of pain meds during surgery, and also some Dilaudid afterwards, so the only effect was that I had a vague sensation that I had to pee. With all of those pain meds, I was able to hold the DMSO for about um, 30-60 minutes quite comfortably (sorry I can't be more specific, the anesthetic & pain meds had also messed with my sense of time a bit). I decided to urinate it out after that, though, not so much because I was overly uncomfortable with it in there, but because I wanted to GO HOME... and they wouldn't let me leave until I proved I could pee on my own.
For my subsequent instills, when I did not have any anesthetic at all, and no Dilaudid shots, basically I clawed at things, cussed at my husband if he was around, and yanked my hair out the entire 20 minutes I was supposed to hold it because it burned quite a bit and made me feel like I had to pee so badly. After the first one, when we actually tried to drive home with that stuff still in my bladder and I almost peed it all over my car's upholstery, I started just camping out in the ladies room down the hall from my uro's for 20 minutes.
Now when I go for one, I take my pain meds with me, take a ThermaCare thing with me, and then after I just sit on the can with a book until 20 minutes is up in case I really do have an accident ;)
So, you could say I thought the instill I had concurrent with my hydro was my preferred way.... ;)
ANYHOO -- many uros do not do any instilling (especially of DMSO itself) during the cysto/hydro, especially if they do a biopsy... instead they prefer to wait several weeks to make sure that the bladder has healed before adding insult to injury. I didn't have a biopsy or anything, so my uro just went ahead and did the DMSO when he saw I had IC.
I bet heparin and marcaine would be much more soothing than actual DMSO....
