Botox cost me $1,400 a vial in the hospital. Hospitals place their own mark up on botox. I believe $600 a vial is a clinic cost.
It took several appeals and multiple coding to get Medicare and Blue Cross Blue Shield to pay. Your doctor has to word his orders just right; then you have to be very persistent and ready to do lots of appealing. You also need a hospital that is willing to work with you on this. Hospitals are not able to share what an individual's coding was because of HIPPA restrictions.
If anyone is considering Botox, I would strongly suggest getting insurance pre-approval.
