View Full Version : How long may I hold a rescue instillation in my bladder?

I am planning to have rescue instills (Heparin 40,000 IU, Lidocain 2% 10ml, Sodium bicarb 8.4% 5ml). After at least one hour I will try to pee it out.

Is there any risk in holding it much longer in my bladder? Should I have cathered it out when I can't pee it out?

before my heparin was recalled I used the heparin, marcaine, and sterial water for my rescue installments. My Dr told me to try to at least hold it for 15-20 mins if i couldn't to let it go for me not to cause myself more pain, but the longer I could hold it the better. over time I got to where i was using the installments up to four times daily. the study showed that it was safe to use up to three times if needed. since i'm not the avg IC person and done it for so many years and got such relief from using it my Dr. said if i wanted to up it to four times if it was helping go for it. I did.
i believe the longest I ever have held it was three hours. I felt like the longer i held it the better. After using the installments as long as I did the pin point bleeding is gone, but sadly my hunners still remain making IC that much more painful for myself.

The one thing I experienced when holding it longer than planned (totally forgot about it being in me lol) was retention...I was told the longer I could hold it the better. I don't believe I had to turn to cathing to get it out it was just one of the more difficult times of retension I ever experienced. I would cath it out if it is uncomfortable and not push yourself to go....do what you can to relax!!
I do think you should talk to your doctor about your concern though since we are all individuals. :grouphug:

My urogynecologist said there was no limit to the time I held the instill. He told me to hold it as long as it was comfortable. I didn't have any problems holding the first one that I had in his office for 3 hours. At first I did the instill at night so I could get a better night's sleep. I was holding it 2-3 hours. Now I don't think about how long I hold it. If I do the instill at night now I can sleep 4-5 hours or longer.

I have never had to cath to go to the bathroom. Just check with your MD. I'm sure he has recommendations.

I was told to hold it as long as possible. My 'record' time is right at 2.5 hrs.

By all means discuss this with your Dr if you have any concerns since what might be safe for one patient may not be safe for another.

I should have pointed out that the time you can hold an instill depends on the ingredients. My cocktail is very similar to your recipe. I don't want anyone to think that it is OK to hold all instills as long as you want. It totally depends on what your MD tells you.

I was told to hold it for one hour, no longer....interesting that we are all getting different directions from our uro.

I am able to hold the 8ml heparin, 8ml lidocaine, and 3ml sodium bicarb. mix for at least 1 1/2 hours or 2 hours on a good day. The longer I keep it in the better.

I have held it about 2 hours or more , yet after a while the urine dilutes the installation chemicals . I would hold it if you can at least 30 to 60 minutes . good luck

I did the Heparin, bicarb, lido 4% instills until recently when the heparin was changed to Elmiron caps X2 contents into the solution. I've slept all night with that instillation many times. Sometimes, the Elmiron seems to burn a little, but rarely.

I had Elmiron instills every week for 6 weeks back in 2005, but it didn't help.

I was told an hour, no more, no less; my uro was worried about retention. I do have some retention problems and waiting more than an hour with the instill does seem to aggravate this (my old uro said 2 hours), so this makes sense in my case.

Hi Peter,

Elmiron was substituted out and Heparin in for mine because of the side effects I experienced with Elmiron. It caused me to lose clumps of my hair. I have heard of many variations of this instill.

What I have heard is that the "rescue instill" is usually Heparin and Lidocaine, Lidocaine and Sodium Bicarbonate. The one that is actually healing the lining is the one that contains Elmiron. I have also heard several variations of that one too. I have heard Elmiron used with Heparin and Lidocaine or combined with Sodium bicarbonate. Also, Cytotec is sometimes used for patients sensitive to Elmiron like myself in place of it. I used Lidocaine, Heparin (40,000 IU) and 100 mcg of Cytotec crushed up in the instills twice to three times a day.

Also, about frequent catheterizing...something else really helped me. I now use catheters coated with Nitrafurizone or Macrobid active ingredients to lessen the chances of developing frequent UTIs. That does not mean I don't get them, but much LESS often.

I do instillations with heparin, marcaine, sodium bicarbonate. I hold the solution for 50-60 minutes. I get no added benefit from holding it longer & in fact, sometimes have burning if I forget & hold it more than 75 minutes. So, totally not worth it. One time I fell asleep & held the solution overnight. It caused burning and I wet myself the entire next day. Not good. But, my doctor says my reaction is not the norm. So, follow your doctor's instructions & know that we're all different too.

Also, as for how long to hold the instills...with the Heparin, I was told NOT to hold it longer than an hour. The one time I forgot I urinated blood clots from my bladder. I usually set a kitchen or microwave timer when I do mine. But again, it depends on your doctor's instructions.

The best thing to do is ask the doctor. Our bodies may react differently than others. I held it again today for 2 hours without retention.
