Recalled heparin aside, the use of rescue instillations continues to be the newest and most popular treatment for IC in the US. Originally developed by Richard Henry MD (Canada), J. Curtis Nickel MD (also of Canada) has released the results of a study that he did at last month's European Association of Urology conference which showed yet more positive results.

Rescue instillations have been my miracle now for two & a half years!
I was very afraid to try them, but they gave me a good quality of life again:)

i have been trying rescue instillations for 3 weeks now, i believe it made a progress wit my pain, but my back pain is killing me, i believe the catheter is irritating my bladder badly, so any suggestions for help as the heat pads & otc pain meds, don't work very well .

Since I'm so chemically sensitive I believe this is a good way for me to treat my flare ups. I'm waiting for a call back from the uro to see if I can do a rescue instill. Uro hasn't encouraged me in this direction though.

Okay uro called back, and due to the recall I can't get a heparin instill, so I'm doing Elmiron, Lidocaine, and sodium bicarb. Wish me luck it's the first one!!!

Hi,
I'm doing weekly bladder instillations of 20 ml of sterile water, 2 Elmiron capsules emptied out into the sterile water(all in a sterile urine specimen container), 12 ml of 2% Lidocaine injectable and 6 ml of 8.4% injectable Sodium Bicarbonate (this buffers the Lidocaine). It helps me a lot.

You can ask to try different catheter types & sizes. My urology resident gave me a "bouquet" of different catheters (I laughed my head off when he came into the room & held them out to me like that) to try out & it really helped me find a good one.
Definitely ask for a non-latex catheter to try. That made a huge difference in the amount of urethral burning after the instillation. I also tried out different sizes of caths. Some patients do better with a pediatric catheter like an 8Fr size (more narrow), but I found I was actually better with a 12Fr. (it was less "bendy", went in easier, & I can instill the meds faster).
If you're getting some relief with the instillations, it just might take some fine tuning of the formula* or the catheters to make them work even better:)
*I found I had less burning with Marcaine, rather than Lidocaine...

thanks Kadi, i''l try to ask next time for more varities in the catheter, hoping that could make the pain better
