View Full Version : Passing blood clots when urinating after instill??

Okay, this really worried me tonight and of course, I can't get a hold of my doctor in Cleveland. I spoke with the Urologist on call, but he doesn't treat IC so had no clue why this would happen. It's scaring me...hence the reason I am awake at 1:30am. I am calling the doctor's office as soon as it opens this morning!

Anyway, I held a bladder instillation in for over an hour with the cytotec, heparin and lidocaine (I got distracted with work)...then urinated it out and saw a few blood clots...but it was nothing compared to my second treatment today. I "thought" I had drained my bladder and did not. I hooked the catheter syringe on the end of the catheter and then up comes bloody, bloody urine and huge clots. Well, it took FOREVER to get the big thick clots out through the catheter and I still don't think I got them all. I am going to have to see tomorrow with Urology down here and see if they can do a bladder wash or something. I can't drive to Cleveland every time I need something.

I was wondering has this ever happened to anyone who instills heparin and lidocaine? I am kinda in my freaking out mode!

I've never had that happen, it must have been alarming!
Calling the doctor is definitely a good idea, hang in there.
Let us know how you're doing after you talk to the doctor, okay?

Hey Kadi,

Yes, I spoke to my doctor's Nurse Practitioner at the Cleveland Clinic and she said if you hold the heparin in too long, it can cause you to pass some clots when you urinate it or cath it back out. However, she said it was NOT normal if it continued after you voided the instillations out.

As for your "Cocktail," mine used to be the same almost and I just used it for pain relief, but now with Cytotec (approved in the UK) to treat IC; it is supposed to help in the reparation process of your mast cells in the bladder, thereby eliminating scarring, ulcers and/or glomurelations.

I have held in my heparin instills with 40,000 cc for about 1 1/2 hours and never had any blood. Do let us know what your uro says. Relax and try not to get too panicky.

Hey bunnykinb,

Thanks for the information. The NP at the Cleveland Clinic told me that holding the instillations WITH the Cytotec for that long can cause the clots to come out. She said it didn't have anything to do with the heparin, really. It's the mixture of the two. Know what I mean?

Anyway, it hasn't happened anymore because I have been so careful to make sure I set a timer or something to keep the treatments in for only an hour or less, depending on the pain level.

I have a regular kitchen timer that has a cord so I can hang it around my neck any time I need to time anything. I got it from a kitchen store --- you might want to think about one of those.

Today, I figured out that our microwave has a timer on it and it beeps really, really loud when it is set. So it's working better this way than watching the clock and trying to remember exactly what time I instilled. Thanks!
