Does anyone use an instillation longer-term (ex. twice a week every week) that has Kenalog in it? My doctor prescribed it but I'm concerned that since it's a steroid it could have a negative impact over the longer term b/c they warn that it could thin skin, etc.
I'm on Macrobid to prevent UTIs. I'm wondering if anyone has an antibiotic included in there instillations. I wonder if it could prevent UTIs in this manner without all the possible systemic effects that taking long term antibiotics could have. I asked my pharmacist about this and she said that since Macrobid isn't a broad-spectrum antibiotic it should be okay to take longer term.

When I was getting instills in the office before I started at home, I went once weekly for 5 weeks. They included kenalog then. But it was not included for home use. I did not question it-I assumed it was because daily use would not be a really good thing. Although, I sometimes wonder if using it once a week or even once a month would help? (of course did I ask my uro at my last appointment? no, duh!). How often are you using it? Daily?
I also use an oral antibiotic to prevent UTI. I was taking macrobid daily for a few months, then he decided I could alternate between the macrobid and trimethoprim. I've been on the trimethorpim for 3 weeks now, and after the month is done I'll go back to macrobid for a while. As for antibiotic in the instill, I've heard of it, I look forward to hearing what others say? I didn't think of asking my uro about that either (duh again). One thing I tried once is a catheter that is coated with an anti-infective material. I got a free sample and tried it, but the funnel end didn't fit well with my syringe adapter. It spilled/leaked the instill all over! I do think the concept of antibiotic in the instill might be a good choice, I have RA and am on some meds that you generally don't take while on antibiotics. My rheumatologist doesn't seem too concerned though, so I suppose I shouldn't be either.

The recipe I have is : Marcaine,Lidocaine, gentamiacin,Heparin and Kenalog. My URO adds the Gent. to prevent UTI's so I do not have to take Macrobid. I started them in 2006 and then learned how to do them myself. Sometimes they help and sometimes they do not . However I do believe they will prevent the flare from worsening . I hope this helps .

I use on a daily basis, lidocaine, elmiron and cytotec and once a week I mix in kenalog with the other stuff, which is very soothing for my bladder and has helped.
