Just wondering if anyone got a handicap parking placard or whatever since being approved? I was checking out our BMV site tonight & didn't see anything on the form in my category to be able to get one. I have a hard time going to the store or anywhere w/o pain in my legs & pelvic pain & it's difficult just to get to the building. Also, did you have to show some kind of proof of being disabled? I have to go renew my license tomorrow & thought I would see about this while there & just wanted some input from others first.

In New Jersey, you can get a form from Motor Vehicle, and it just has to be signed by a doctor. I went with the option "Unable to walk more than 200 feet without having to stop and rest." (Not strictly true, but certainly unable to walk any distance at all without pain...) I had it signed by my psychiatrist. I got the placard. I think they'll give it to anyone. But this varies by state, so I don't know what Indiana requires.
I love my handicap placard. It just gives me that little bit of extra confidence, that I can go out shopping or whatever and even if I'm exhausted and in pain at the end, at least I know I don't have to trudge for miles through the parking lot.

In Oregon you get the application form from the Motor Vehicles Department, fill it out, and have your doctor sign it. I picked up the application, but I don't qualify. It has the 200 foot statement and I walk down and back a 450 foot driveway twice a day to pick up our paper and mail.

In Texas its the same I think, go to the MVD and pick up the form, I have chronic pain plus other illnesses so the doctor who signed it (which was my pain doc) didn't have any problems doing so). I don't use it all the time, but its there when I need it.

I picked up the app at our sec of state, and had my doc fill it out, and went back and picked it up.

I live in California and obtained the form through the DMV website. I took it one time to my primary care doctor and another to the uro for a temporary placard which is good for 6 months. My primary care doctor said that it is difficult to get a permanent placard for someone at my age unless I couldn't walk.
