If you have IC symptoms and respond to IC meds and have normal bladder capacity, would a hydrodistension to only 500 ml be enough to show that pinpoint bleeding and the gloms... (sp?) that they say is indicative of IC?

Possibly. It all depends on your individual bladder. Unfortunately, there is no predictable distention volume that will produce glomerulations. Some bladders show them at just 200ml, others don't show them until 800ml.

Now, say with normal bladder capacity and the uro distended to only 500 ml and didn't see anything abnormal yet, do you think that he/she would/should continue to distend a little farther to see what he could see at a higher volume of distension?
But uro said he didn't distend beyond 500 ml because he didn't want to rupture the bladder. Sounds questionable to me. I'm asking these questions for another person based on the info she told me the uro told her.
I have normal bladder capacity and was distended to 1275 ml and had urinary retention of 1000 ml afterwards, extremely painful, but my bladder didn't rupture, though it sure felt like it was going to.
Remember I'm basing this on a person with normal capacity bladder. I know everyone's different, just trying to figure this out a little.

Normal bladder capacity is about 500 cc. So if 500 cc of liquid were put into a bladder with a 500 cc capacity, the bladder would not be distended at all. Did your friend have this procedure done in a hospital with anesthesia? It sounds more like a regular cysto, usually done in a doctor's office.

that does sound like an office cystoscopy rather than a hydro. You can normally tolerate double awake capacity under anaesthesia.
Saying that, had urodynamics and normally have capacity of 200 on a good day, and got to 400! excruciating to say the least. Had a hydro recently- would be good to know how much more they got - I'll be suprised if much more than 400-500!

That's what I thought, but I was given the impression that this was done in a hospital OR under general anesthesia. I'll have to ask her again to be absolutely sure..

Urologists will often take photos of the bladder during the procedure. These photos are included in the patients chart. This is a quick way to answer your initial question. But, you may not want to view the photos because they can be upsetting.

Oh no, not upsetting for me. I demanded they make me my own copy. I'm a science teacher and don't easily scare :) I thought they were very interesting! I asked for a copy because when I had a laparoscopy in the dx process, I was shown the pix but not given a copy of my own. I was bummed. I find it really fascinating to see what my innards look like!:rolleyes:

I thought in a hydrodistension the bladder is distended to a certain pressure and that is how they know what the capacity is / when to stop.
?!!

Donna.....My bladder distended to 450cc. Is normal 500cc under anesthesia?....I thought my doctor said that 900cc was normal. Just curious what "normal" is.

My doctor stretch my bladder as far as it can go. I thought this is the way they can tell. Maybe there is a reason why they aren't doing this now, maybe you can ask why.

You can have IC and have normal size bladder. I do. Frequency and urgency I have is due to inflammation of my bladder. 450cc is close to normal WITHOUT anesthesia, like Donna said, normal being 500 cc (ml)
900 cc would be something done only under anesthesia...extremely painful while awake.

I just had my Post-Op today from a Hydro/Cystro on the 12th.
She told me that normal bladder capacity would be 1000cc.
Mine is currently 750cc. It was 650cc back in 2003 when I was first diagnosed.

Just wondering why you had another hydro? Also, have your symptoms improved along with your increased bladder size?

I had the follow-up hydro/cysto due to the fact that the last 3 years when I go to my Uro I have microscopic bleeding (hematuria) in my urine sample. She thought it would be best to be proactive and make sure that we weren't missing something other than the obvious.
Since I was diagnosed in 2003, my symptoms have really been improving I would say every year. All in all, I was a bit apprehensive about having this procedure done since I really only have about 1 flare up a year and didn't want to have anything worse start up again (been there, done that!). The increase is probably another reason (besides the meds - 20 mg. of Elavil before bed, Elmiron 200 mg - 2 times per day) I don't get up in the middle of the night to urinate anymore. :woohoo:

Thanks for your response. Your story is encouraging and offers me hope. I was only diagnosed in Oct 06 (symptoms since July 06). My hydro capacity was only 450cc. I am on Elmiron (300mg/day and Elavil 20mg/day). I am still getting up 2 times/night and going 8-10 times/day. However, one of the times I wake up (usually 3:00am) I think has become a habit awakening. I'm going to try to ignore this as an "I gotta go" time and retrain my bladder. ?????? Anyway, thanks again!!!

Hi everyone,
When I was diagnosed in 1990 my bladder capacity was not normal but I had a hydro in 2005 because I have been having horrible pain with flares but my uro told my mother under anasethia it was now a 1000 which was good. I have my pictures from my hydro and it still shows pinpoint bleeding. He said that patient with good capacity did not do good with bladder removal because they would still have pain. He said to be a candiate for bladder removal he would only do it with bladders that where small in capacity as they benefit from it more. I have built up my bladder capacity by waiting to urinate as long as I can when I am having good days and it has worked for me but you just have to make sure you get all of the urine out of bladder because it will be come stagnant and cause a UTI. When I am in horrible flareups I urine as need I don't try to hold it on these days. My main problem is pain everyday. I have alot of urgency and frequency in a flareup. Well have to run.
