View Full Version : Emergency Care Instructions for my Ostomy and general health!

The following information is all personal to my own health and care! It's just used as an example of what kinds of things I put down that were important to my health.

I finallly ordered my Medical Alert Bracelet. It should arrive here in a few weeks. It was quite expensive but hopefully it will be worth it someday. It's a sterling silver, classic style, heavey cable chain, with a Hex Blue Alert Charm attached to the end, it has a claw clasp.

It has my phone number, KARA L. REESE, CONTINENT URINARY DIVERSION, CATHETERIZE ABDOMINAL STOMA, EVERY 4-6 HOURS WITH A 14 FRENCH CATHETER 16 IN. LONG (SEE PATIENT INSTUCTIONS)

It was a bit diffcult because I could not fit much on there and it had to be limited but none the less, it has the most pertinent information that anyone would need.

I've also put together some personalized Emergency Medical Information. Allergies are a big thing of mine so I have to have this first on my Emergency Contact Form.

I put down what I was highly allergic to: I am highly allergic to:

Morphine
Demerol
Methadone
Compazine
Anzemet
Reglan
Bactrim
D5LR

I added that I have problems with anethesia and am resistant to Versed

Please only use Zofran for Nausea and Vomitting.

I added all of my PRN as needed medications

I put down what medications I am currently taking and the dosages. Lyrica, Keppra, Clonazapam, and Inderol. I also put down that these were subject to change.

I added that I have Celiac Disease (explained it) and that my diet is:

Potatoes of all types
Alll Meats
Rice
Rice Crackers
Salad
Vegetables
Fruit Punch
Lemonade
Milk
Rice Crispy Treats
Lemon Juice
Salt and Pepper

Everything was done separetely and I had it all laminated. I had 3 copies made. One for the house, car, and my purse.

Keep in mind this is specialized personal care regime and only made up for myself.

EMERGECY CARE OF URINARY DIVERSION and CONTINUED CARE OF URINARY DIVERSION WHILE IN THE HOSPITAL

"I have a Continent Urinary Diversion which is how I empty my urine. My stoma is located right next to my belly button to the right, this is where you slowly insert the catheter to remove the urine (you may need some lubrication to insert the catheter) if no urine comes out you may have to slide it in and out a bit. There is a little resistance point in there where if feels like the catheter may not go in at all, gently push past that point and urine should flow. If no urine still does not come out, you may have to put some saline in there with a Toomey Syringe, try 20-30 cc’s of saline and then gently pull on the syringe, you may see some mucous or urine in there. Take out the syringe to see if any urine comes out. If not, please try again, repeat with 10 to 20cc’s of saline with the Toomey Syringe, DO NOT PULL TOO HARD ON THE SYRINGE! Please Catheterize every four to six hours to eliminate urine with a size 14 French Catheter, 16 inches long. If you have just found me, please catheterize me immediately as you won’t know when the last time I went was. You can damage the tract/opening by introducing a catheter that is too large! "

NO MRI I have a MEDICALLY IMPLANTED DEVICE by Medtronic the ID # is in my wallet.

I put down my Major CONTACT people and Emergency Numbers.

I will have an emergency Ostomy Kit in a small duffel bag in my car. I also have some emergency supplies packed where ever I go when I go away.

The RED emergency folder will sit on my passenger seat. The front says in Black, Emergency Care Information for Kara Reese.

Wow! I am impressed:) . I know that there are some medic alert bracelets or necklaces that instruct the person to call a specific number to get all your info. I do not wear a bracelet or necklace, BUT I do have a card in a an open window on my wallet that says I am on Coumadin and it also says Continent Urostomy- cath every 4-6 hours witha 14F coude catheter. It also has my dr's number on it. When I travel I always carry the discharge summary from my last hospital admission, thus giving vital info. I used to ahve this really cool little thing that you looked into that had all my info on it, but the hook on it broke and I just never got it fixed. I used to keep it on my key chain, oh well.

I have all of this information on 4 by 6 Cards in my purse. I have a Medtronic Card in my Wallet with my Surgeon's name on it. Iam working on putting together an extensive ER Ostomy kit in my car that is in a duffel bag and can travel with me. I already had one for my trips to NYC but I've taken some things out of it for use, so I need to go over it all again.

You're every EMS personnel's dream patient. Those cards and bracelets should come free from our doctor's the very day we are dx'ed with a debillitating disease.

The day I first had my bladder removed in 2002 the doctors office should have had something made up for me. At least a little wallet emergency card. I had to figure it out on my own that this was needed. Grrrrrrrrrrr. But at least it's done!

This again illustrates how everyone's doctor/ET nurse is different. When I got my cystectomy I was given info on how to get a medic alert bracelet/necklace. In this day and age the patient is responsible for their own health care, the doctors office can give you the info about how to go about things, but ultimately you know what info you want and need to give out on a card or medic alert. Of course if you are uncertain by all means ask the doctor what you should include. I personally prefer it that way, as when I was younger what the doctor said was what you had to go by. There was no way you could question or be a part of decisions in your health care. In fact some doctors did not even tell their patients when they had a terminal disease! So even though now it means more work for the patient, it really is better this way.

I have had my bracelet for 2 months now...mine says:

CONTINENT URINARY DIVERSION, CATH ABD STOMA, EVERY 3 HOURS W/16 FR. COUDE CATH. HTN. NO MORPHINE.
Medicalert also give you a laminated wallet card with this info and your dr, emergency contact,etc..

Kara, do you have Interstim? It sounds as if you do. If so, are you keeping it even though you've had the diversionary surgery? On Monday, I asked my uro to take it out since we were talking about the cystectomy. Does it do any good? Or does it even have a purpose anymore?

I still have my Interstim, yes. We are not removing it just yet as it may serve a purpose with the nerve pain I still have. It will need to be revised of course. It is not working and turned off at the moment. You never know what is around the corner with todays technology. They may be stimulating me bilaterally this time around if I keep it. For some reason I have not been wanting it removed. Maybe the reason will become clearer as more testing is done.

So you are having a Cytectomy? Is the date scheduled yet or are you just talking about it. Are you having an Indiana Pouch put in? If your Interstim is in your abdomen, you will most likely have to have it removed. If it is in your back then you could possibly keep it.

My uro told me I really didn't have much of a choice. He said I could have the cystectomy or move into the bathroom and work from there!
I am just having a hard time thinking about even needing this type of surgery. Then the thought of having a pouch or bag just blows my mind. Other than IC, I really have nothing else wrong with me. And I've never been really sick, so psychologically this is doing anumber on me. I'm in a lot of denial.
My interstim is in my back. I suggested he take it out and he looked surprised. Instead, it was reprogrammed.
I have no date. I don't have the time to do it before August. He suggested June so I would be ready for school in the fall - but I don't need to be since I will be between jobs.
