View Full Version : New Uro wants me to do rescue instill tomorrow!

She seemed nice, very knowledgable.... she was impressed about how much I knew....

She reccomended to do heparin, lidacain and bicarb with saline. She said I could come tomorrow for it. She will do the first one and then her nurse could do the rest for me. She reccomends 3xs a week this week, 2 next week then taper off to 1 a week.

I have been in a rancid flare since my laproscopy 2 weeks ago.... the pain is better, but I am having a hard time sleeping from the pressure and frequency.

She suggested instill cause i have been on elmiron for 3 years and am not in 100% remission.

My only concern, I have a business to run. I have to work regradless in two weeks. I can work the way I feel now, but if I am any worse I have no idea what I will do.....

Could the instill make me worse? The new uro said there is always that chance but she said all her patients do wonderful with it and it helps their pain.

She also will check my stream and make sure I am emptying tomorrow. She also said she would check to make sure I didn't have urethrea retention.

Any thoughts?

She also said no to pre antibiotics for possiible UTI.... I will ask again and request for one, for the last 2 caths I have had for surgeries I have had an infection. I have tons here I guess I cold take it with out telling her!

I think you're worrying too much! I just got done with a series of six weekly rescue instillations. They didn't help, but they didn't hurt, either. Most patients seem to benefit a lot from them.

At least twice I went straight to work after my instillation. I don't think you will have any problem going back to work.

I didn't take antibiotics. I think if they're careful at your doc's office, and use the correct sanitary procedures, it shouldn't be a problem.

I do rescue instillations at home about every other day. They have restored my life to some semblance of normal.

If the first one goes well, perfect!
If the first one burns, you can ask your doctor about non-latex catheters, amount of water dilution, amount of hold time. So, if the first one gives you some relief, but has a few problems, they can hopefully be solved!

I am much much better on them than off. Hopefully you will have similarly good results.

(I also do not take antibiotics with them, have had only one short infection in the 9 months I've been doing them.
Since I'm allergic to 5 different classes of antibiotics, I try to not take them unless I have an actual infection and preferably a culture/sensitivity to make sure they are the right one. More exposure=greater chance of new allergy, so I don't take any antibiotics unless actually necessary. BUT, I'm also not prone to bladder infections. Have had only one since the age of 5, and I'm 38 now).

Leanne - I would ask for marcaine instead of lidocaine. It lasts longer. Also, no matter how careful they are at the doc office and how sterile everything is, there is a much greater chance of getting a UTI, because of the bacteria hanging around on you, not on their equipment. Also, sometimes there is urethral discomfort with instillations. Over the past three years, I've had about 35+ instillations, with no problem. I went on a 3 x per week, 3 - 4 week treatment plan and after five instills I got a raging UTI that has taken three weeks to get rid of, with three series of antibiotics and numerous Diflucan. I'm a little apprehensive starting things again. Just thought I'd share. Some docs will give a prophelactic antibiotic (1) when they give you the instill to ward off potential UTIs.
Don't mean to concern you, but want you to have the info from different experiences.

Well, I have spent my entire day yesterday researching this.....

here are my concerns:

1. I don't think I am at my end wits with this disease. I don't think I am at my possible worse in pain and symtpions. Even though this has been a hellish flare broughtt obn most likely from surgery, stress and the cathedar-uti.... I am getting better on my own.

2. I want to reserve the option for "rescue" installions for when I need the rescue.... right now I am getting away with living my life and going on each day, but with a mild reminder I do have bladder prolems so I must make sure I take time for me.

3. I went to my yoga class last night was I left feeling much more in control with my body.

4. I started my period this morning, so there is now no way I will let anyone touch me now! I'd rather have sex with DH than get cathed!

It must be a pretty strong fear I have, but I see rescue installlations as my end all option for pain relief, and think I have some other options before I must use that choice.

Considering I have not used pyridium or uristat since friday so I could get a clean urine to the new urologist, and now its so much quieter today than it was yesterday.... and this weekend.... so I have hope its going away.

Thank you for my your support... I definantly would love to hear more cause I know that is is an option down the road when I need it.

I figured with what I do for a living, if I am flaring bad and I know I have a 10 hour wedding to shoot, I could get one the day before and hope that I will get enough relief from it to work comfortably, instead in pain.... but thank goodness my work is only 6-8 hours infront of the client and the rest in the confort and convience of my own home.
