First off, I want to thank everyone who replied to my first post a few weeks ago. This seems like a wonderfully supportive network of people, and I'm so glad I found it.

Two weeks ago I had a cystoscopy under general anesthesia. The uro did a urethral dilation, a biopsy, and hydrodistention. He found nothing wrong and has now basically written me off. I'm not sure what I should do next and need some advice.

Medical history: About 8 years ago, I had recurrent UTIs--4 of them in as many months--and because I was only 19 and stupid, when my GP said the word "urologist" I freaked out and did nothing when the UTI came back. The symptoms went away after two weeks and then stayed away for a long time. A couple of years later, I experienced what I think was the first instance of what's bothering me now. It felt like pain/pressure in my bladder and a constant urge to urinate even when empty, but no burning when I did go. I had no insurance, so I took Uristat until the pain went away. The same happened a few months later. This was five years ago, and since then I've had urgency all the time--that is, one minute I feel no need to urinate and suddenly I have to go immediately, and when I do go, there's not all that much coming out.

The urgency intensified this March and suddenly I was having the pressure/pain again. Doctors could find no trace of a UTI (but put me on antibiotics, anyway  ). In August I had two episodes of severe lower back and abdominal pain, the second one putting me in the hospital for a day while they made sure it wasn't a kidney stone or appendicitis. More doctors, more antibiotics, no relief.

Two months later, and I still haven't found anyone who can help me. Like I said, the uro has given up. He said he found nothing abnormal during the cysto, and all he offered me was "some medication" for the urgency. I don't feel at all better since the procedure (if anything, I feel worse because of increased pain), so whatever the problem, it's still there.

So far, the only physical issues they've found are blood in my urine and an elevated white blood cell count (this on the day I ended up in the ER and was admitted to the hospital--both issues had cleared up by the next day), and when the uro tried to do an in-office cysto, he couldn't get the scope in. My urethra is apparently unusually tight (and the opening, I guess, is in a weird place--hidden, not easy to find). He said my vaginal opening is also unusually small. But he said nothing more about this, so I have no idea if it's significant or not.

I have other problems right now that may or may not be related to this. The back and abdominal pain has persisted. I seem to have flare-ups where it's severe for a week or so and then eases up a little but never goes away. I have to use a heating pad on my abdomen every night, or I can't sleep. Two or three times a week, I wake up in the middle of the night or early in the morning with agonizing stomach cramps, acid indigestion, and/or nausea, which usually ends in a round of vomiting. I'm seeing a GI specialist for this after the weekend.

Another problem is recurrent yeast infections, which I've had for 6 years. I can count on one hand the number of weeks during these years when I haven't had yeast or bacterial vaginosis. Every time it's treated, I get it back within a couple of weeks. My OB-GYN is stumped. He sent me to an infectious disease specialist, who's also stumped and told me I should try the Mayo Clinic or the Scripps Clinic for help, because this is totally abnormal and my only other option is to take Diflucan for the rest of my life. I'm undecided on what to do about this, and right now, my back/abdominal pain is more of a pressing issue.

So it seems as though my options are running out. When I was told I might have IC (by the infectious disease specialist), it made a lot of sense, but my uro seems to feel he has ruled that out. Would there be any point in going to a different uro for a second opinion? And other than a wild guess, how can I find one who might know more about IC and be able to help?

Any advice would be appreciated!

I personally think I would definitely try another uro - and if that doesn't get you anywhere, I go to another etc. You've had one specialist bring up IC and one say he didn't think so...I would definitely look for the "tie breaker"!! You need to find out what the heck is going on, one way or another. But I do know it can be extremely frustrating to find a decent doc who understands and is interested!

I am sure others will chime in with ideas and whatnot, but I just had to throw in my 2 cents! ;)

Hugs!

There is no one definative test for IC at this point so it is definatly worth seaking a second opinion.

I would ask for a Potassium test if you do so. Don't give up. We are here for you.

I agree with everyone - time for a second opinion. I hope you find answers soon and will feel better soon.

:grouphug: I'm not sure which uro you saw in LV, but the Desert Urology practice on Shadow Lane near Mountain View Hospital is awesome.

Thanks, everyone. I'll definitely try and find someone else. I've almost hit the max on my deductible, so from now until January, I've got a free ride. Should definitely take advantage of that, eh? :)

Babs, thanks for the tip. The first uro I saw (but abandoned because of how long their wait time was for a cysto) was Desert Urology in their NW office. Can you tell me which doctor(s) you've seen in the Shadow Lane office?

I think I would seek a second opinion too, it never hurts.. keep us posted on how things are going.

Sure, it's Dr. Gregoriev and he is a wonderful, caring, sympathetic guy who actually listens and asks for your treatment goals as well. Which part of LV do you live? My hubby grew up in Vegas and graduated from Bishop Gorman. I actually lived near Seven Hills for a time before hubby's deployment orders got changed and worked at Mike O'Callaghan Hospital at Nellis.

A few more things to try. It does sound like IC to me, though there are a few other things you can take the initiative to rule out, since you have met your deductible anyway! Since you do seem to have symptoms of a UTI but with bacteria found (back pain, wbc and red blood cells) ask to get tested for ureaplasma and mycoplasma. They are bacteria that need a special type of urine culture done to test for them. I got my test through my naturopath, I requested it and she ordered it. It came back negative for me, but she then had one other client come in with symptoms of IC, and she ordered the same test for her and it came back positive! You can also get a urine test for candida, believe it or not, to see if you have an infection in your bladder with yeast! Highly rare, but it still can happen.

I am just giving you options to ask for if you wish, just to rule more things out. However, it is possible to have IC without it showing up as glomerations during a hydro. I have heard of that. I think you should get another opinion. At least, if nothing else come of it, just ask if you can start on some IC medications like Elmiron to see if it helps you....if you are in so much pain there is not much to lose.

If I was your uro I would guess you had IC just on symptoms. Don't give up! I had to go to 4 doctors before I was finally "officially" diagnosed, though each suspected that I had IC from my symptoms.

PS, I like your name, legalalien. My husband is a legal alien too! (He is from England.)

Babs, thanks for the doctor's name. I'll definitely check into him. Right now, we live in apartment at Peccole Ranch near Summerlin, but we just bought our first house (should be done in January, unless it keeps raining!), which is at Aliante in NLV, so not too far from Nellis. My hubby has lived in Vegas all his life, too (I'm from Norway--that's where the legal alien comes from :)--we met on the Internet).

massagedoula, thank you SO much for the additional info. It'll help me out a lot to know what I should ask the doctors to do!

It is just an idea. They might look at you like you are crazy...but it is worth a try to ask! Most likely everything will come up negative, and you do have IC all along, but it is worth ruling everything out!
