View Full Version : Questions about instillations of Heparin & Marcaine

Hi, we exchange some info the other day before I went for my training session with boyfriend to learn how to do instills with heparin (2 vials) and marcaine (1 vial).

I would like to step back to give more detail - Mid February thru Mid March, I had 3 bad infections. So that prompted the Doctor to want to do a Cystoscopy with Hydraulic distention, which she fould broken blood vessels and these marks on the bladder that looked to me like stretch marks (thick white lines). After the procedure she did the 1st instillation of the heparin and marcaine. Then for my two week check(4/15), we did the learning instillation with boyfriend.

We are okay doing this, but I finally got all the equipment needed and on Wednesday(4/20/05 @ 9:40pm) we did the instillation for the 1st time at home. He did well doing this, but it seemed different to me. I felt a little crampy maybe pressure. But I was under the impression that the longer I held it the more time the medicine would have. So I went to bed, in the morning I went to bathroom and only went about 6 to 8 ozs. Which I thought was odd, then I was on fire the rest of the day and it has continue relentlessly even now. I have taken 1/2 Loricet plus and AZO Standard.

So how long should I hold it? and Do you ever had tissue stuff in your urine?

I was told 15 minutes to hold it but if you can hold it longer to try but not to cause pain

In the beginning I found if I held it too long (15 minutes is what I was told too) the pain was worse and sometimes I could not pee afterwards. I found it did better holding it just the 15 mintues. Now years later I can hold it 1-2 hours and find that it helps more that way. Maybe the bladder needs a small build up to the heparin? No, I have never had any of the bladder lining in my pee.

my doctor said 20 mins is long enough but you will notice with each treatment you should be able to hold it longer, but however you could have gotten an infection and you need to be checked. if you do have a u.t.i you can get on an anti. and start feeling better soon

OH MY!!!....I was told 30 minutes TOPS!!! Maybe that's just for me?
I couldn't go all night without peeing, let alone urine and the volume of the instilled meds. Did you ask your doctor how long to hold the meds in? It may have numbed your bladder and everything around it so much, that you couldn't void. You should at least be getting out all of the instilled meds. Because I don't always empty out all of the way (anyways), I have to measure the fluid voided out to make sure I get all the instilled volume. If I don't, I have to straight cath the rest out.

MY doctor told me to hold it as long as I could 1-5 hours..., I took him the post of different "brews" today and he questioned about 30 min and felt it wasn't long enough! I'm not sure what would cause you to retain, maybe you should call your dr.
(I had a really plesant appointment with my doctor today and he really appreciate all the "brews" people wrote in. He is very open, doesn't claim to know it all but really seems to know alot about IC!)

I agree that you should be checked to be certain you don't have an infection. Any time we have a catheter inserted there is some risk of infection.

When I was on heparin instills, the instruction was to hold it for one hour.

I wonder......why the difference in the 'holding' time of the instillations? Could it be just the doctor's preference? Maybe the kind of meds used? Maybe they start you on the least amount of time, then increase it if needed (what will work for each individual?)?

:idea: This makes me wonder that if I can hold it longer (than the 30 mins as suggested by my doctor, which I'm sure that I can do if I have to!), would it control my flares faster??? (As I'm in a flare that is trying VERY hard to escalate.) Just a thought I had after reading everyone's holding times.

Any thoughts on this from anyone?

My doctor believe the longer you hold it the better it works. I have read you are not to hold it untill you are uncomfortable or in pain, though.

I have held instillation fluid in for as little as 30 minutes to as long as two and one-half hours. It does not seem to make a difference whether I hold it for a long time or a short time. Now I just retain it until I need to void, as long as I am comfortable.
